"More than 20 years ago, I founded this home health agency with a simple but deeply held belief: care should feel personal, consistent, and grounded in genuine human connection. From the very beginning, my goal was not to build the biggest agency, but to build one that families could truly trust.
Over the years, I have seen firsthand how receiving care at home can make a meaningful difference. Healing happens best when people feel safe, respected, and known. That is why we have always focused on knowing our patients by name, listening to their stories, and tailoring care to what truly matters in their daily lives.
As a family-owned agency, we take our responsibility seriously. Our clinicians are experienced, compassionate professionals who are committed to delivering thoughtful, high-quality care without rushing or cutting corners. We work closely with physicians, hospitals, and referral partners to ensure clear communication and smooth transitions from hospital to home.
Today, as we continue to serve communities across the Bay Area, our mission remains the same as it was on day one: to provide personalized home health care guided by experience, integrity, and compassion.

Founded in 2001, our agency was built on a deep belief that high-quality health care doesn’t end at the hospital or skilled nursing facility door.
We bring skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work, and home health aide services directly to patients across the Bay Area.
Our story began with a mission.
When Stanford Hospital closed its home health department, our founder, Lorna Beukema, a clinical manager there for over 15 years, chose to continue that legacy of excellence in the home setting. Alongside seven of her former Stanford colleagues, she launched Nursing & Rehab At Home to serve patients recovering from illness, surgery, or injury with the same high standards of care they'd received in the hospital.
Over 20 years later, we remain a trusted partner to hospitals, skilled nursing facilities, physician practices, and families, delivering personalized, Medicare-certified care to patients throughout the region.
